Default d5 intercooler

2002 V70 D5 108000 miles
I have recently had to replace a spilt hose leading from the turbo. I then found the intercooler was split and replaced that. This solved the poor performance and clouds of smoke problem but the car still runs very roughly at low revs especially when cold, it sounds like a taxi and is still using too much fuel (was 50mpg and is now 40mpg). It does not smoke any more and seems to run well as the revs get higher.
What should I look for that can be causing this. Please advise.
