When you say the braks failed do you mean the pedal went hard as in very hard to press?
If that was the case it is a common fault on the early s/v40's and it would appear the only cure is a new servo. |

check the brake pipes inthe rear wheel arches (common muck collecting /corrosion point) if no obvious signs of leakage and fluid level is normal suspect master cylinder seal failed.

possibly the master cylinder then , the secondary circuit comes in at about 50% travel so you should be able to stop . has the brake fluid been changed every 2 years ? It should be fairly clear with a yellow tint . does it still go down if you keep a gentle but constant pressure on the pedal ?

Also check the pipes around the brake master assist cylinder, the hose might be leaking or indeed the brake assist diaphram meaning you don't have the most effective brakes - what car is it as if its a diesel it might be something to do with the vacuum pump.

if it is the vacuum pump the pedal will feel rock hard with no travel and you have to push 4 times harder , the diesel and auto petrols have a vacuum pump .
