In the recess where the Adblue and diesel filler caps live there's a little white drain in the bottom. Seems this can block easily. I only noticed last week when I went to top the car up with Adblue and it overflowed, filling the recess and unhelpfully appearing to drain straight into the diesel tank thanks to the capless design. No idea how long it's been like this (18 month old V60). Now beginning to wonder just how much water has puddled it's way into the fuel tank over the winter.
