I think most underwriters/insurers use a sliding scale of hp increase to assess the risk. One of the boundaries is around 10 hp increase. I am not in the business so this may be way off. No doubt someone with better knowledge could comment.
I am with LV but with a T5 the optimisation only increases max hp by 3. LV only asked me what increase in max hp is acheived with the optimisation.
Thus I don't pay any extra. However, it deffo shows as an engine mgmt mod on my schedule.
The other issue is that most underwriters/insurers are not technical so some may just say no to any engine mgmt mods.

I take delivery of a new 22 XC90 next week on which I have specced Polestar so am looking now at insurance. I generally use NFU and Admiral for various insurances and have spoken to both. Seems to me if Polestar was fitted by Volvo or their main dealer before delivery and it shown as such on the vehicle documents then they are happy, its when it is fitted post delivery that more questions get asked. I went with Admiral in end and their customer relationship advisor (or whatever he is called) seemed a bit older and more interested than most and there was some discussion around Polestar to the point he actually went onto Volvo Polestar website and read about it. Made a comment along the lines of ‘I can see it enhances the drive ability of the car as opposed to increasing the performance of the car to any great degree’ said he was happy to confirm it will not effect the premium but he would specifically note it on the schedule, along with the side steps which were also fitted at the dealer as opposed to the factory.
