Sounds to me like it is just the pulsing of the fuel pump itself, if there is anything wrong then it would probably be the fuel pressure regulator (FPR). The FPR's do wear over time and mileage and become weaker with age.

At the moment as long as it is not annoying you and there is no major change in MPG or performance I'd be inclined to no worry about it.

I agree with that, once the routine service has been done, it would be top of the list for components that can cause trouble, although I'm surprised it has not thrown a fault code.

Just been on a 100mile round trip and the pulsing on the motorway in 5th at 2300rpm/75mph felt more like a flat spot. If you hold it at those revs then it pulses, if you accelerate past it it has a flat spot feel like just before your turbo kicks in.

It drives lovely other than that, I was concerned about it smoking as it isnt always sure black smoke sometimes its lighter but never on tick over so not sure whether to worry or not. If the turbo was going then how much smoke would there be?

With smoke:

No smoke:

Grey smoke can also be caused by a head gasket failure, but most turbos make a noise when they start to fail and often leak oil. If a car has been in storage for some time and the oil feed to the turbo is blocked or the bearings corroded, it might be a silent failure. I had one do that on a Volvo Penta once, no noise, no smoke, just a loss of power. If you don't have a hiss or whine I doubt if it's the turbo.
Your symptoms are rather confusing, but I would rule out the filters and EGR first. A blocked EGR will cause a flat spot and you can get an idea by just removing the intake hose, because if it's dirty, then the EGR is also going to be in need of a clean.
