Default Misfire when cold. Engine surging / hunting

Hi guys. Fairly new here, looks like a good place.

I have a few things i need to sort with my 2000 S80 T6.

I'll try and be brief and to the point, I have a misfire problem for the first few seconds when starting from cold. It's generally when it's been left over night. Seems a little worse when it has been left for 2 or 3 days. Misfires for a few seconds and then it's fine. When the engine then idles it tends to surge a little. Not enough to show on the rpm but noticeable with the engine tone. The lights are flickering at this point too. (Headlights, interior lights, dash lights etc). It's done 119k miles.

It drives perfectly, just noticeable, sometimes the light flickering is worse than others but the engine surging is always there.
