Unhappy '08 S40 2.0D Hesitating/juddering around 3000 RPM

Hi, recently purchased '08 S40 2.0D, which seemed fine; then the 'engine system service req'd' message appeared and the car went into limp mode. A visit to Volvo dealerwas fruitless, they could find no error code. This happened several times, got the message, car limped home, would be fine after turning off and restart. Dealer changed fuel filter to no effect. Since then, the error message no longer appears but another problem has become evident; driving uphill in particular, dropping the car into second gear, the engine started noticeable judder or hesitation whilst accelerating at around 2800-3000 revs, same in third. The car is not kangarooing violently but the power is not be applied smoothly - as though you're lightly feathering the gas pedal is the best way I can describe - so it sounds and feels like the power is off/on off/on etc.. There's also a kind of 'shushing' sound accompanying the gain/loss of power. Back to dealer, again, they replaced the diesel pump (unheard of, so I hear) which they said had a hairline fracture - BUT this still hasn't solved the problem: any suggestions/clues gratefully received.
