I have owned a xc90 d5 2003 for the last 2 months. It's got 108k with fsh. As soon as I got the car these was a hissing noise. Found out it was a split air pipe attached to the turbo. Replaced it and got rid of 50% of the hissing noise. Then after putting in my first full tank I got 260 miles before the light came on. I done some research and cleaned the turbo control valve governor they call it. There was a complete loss of power driving as if it had two cylinders. Had it checked ou at the garage who done a injector flow test.3 injectors needed to be placed. I had all 5 reconditioned and the car reading went from 24mpg to 30mpg. It was more responsive from the get go. However my mechanic drove it and said its still does not run right. When I put my foot down it feels like the engine wants to go but its being held back. I can feel the engine vibrate when you give it some. Over two weeks it got slower and more sluggish. I cleaned the maf changed air and fuel filter, gearoil changed checked intercooler and still no change. I can't see and leaks in the hoses. Put it on a launch diagnostics with no faults. Then I hammered it out of frustration and it went to limp mode. I thought it was already in limp mode it was that slow. The message engine service required showed up. I turned the engine off andc on went out is limp mode back to the normal slow Volvo. I put it on the diagnostics again but no faults. It has not come on since, but still sluggish. Then I noticed a hum hum hum noise from the engine. I disconnected the maf and noise went away. Drove around and was in limp mode. Connected back no limp mode but remained the same. By the way the engine was sluggish before I cleaned the tcv. It's booked in for a smoke test next week. Before I attempt to clean the egr valve have you guys got any ideas what it could be? Sorry for all this writing.
On reading your post, and even before I got anywhere near your last couple of sentences I said to myself 'classic EGR symptoms'.

My money is on the EGR, a good clean out should do the trick. I believe on your model you can quite happily blank off the EGR will no ill effect.

I'd whip the EGR off give a good going over with carb cleaner and then drop in some decent fuel plus additive then give the car a good old blast and really give it some (once fully warm of course and not in 1st or 2nd). I have noticed over many years that those who drive their motors spitively seem to have less problems when it comes to EGR valves.
